Graduate Transcript Requests

Commack High School has partnered with Parchment to order and send your transcript securely. Please click here to get started.

• Please note, there is a $7.00 fee for each transcript requested.  Payment will be processed through the Parchment website.

For all other alumni records please email Marilyn Shoemaker at mshoemaker1@commack.k12.ny.us.

Provide the following:

• Record requesting
• Name and year of graduation, (North or South if prior to 1988)
• Email address or postal mailing address to send document

Education Verification Information
Commack High School has partnered with Parchment to process student Education Verification Requests. 

Please ensure the year of graduation and the name used while at Commack High School is included with the request.  For all graduation years prior to 1989, please indicate North or South.
A signed release is required for ALL education verifications.  

There is a $25.00 fee for all verifications. Payment will be processes through the Parchment website.
